Astellas Ride of A Lifetime Contest Rules
- 1. Astellas Pharma US, Inc. is sponsoring a contest in which five (5) organ transplant recipients will win a trip for two (the winner and one guest) to Pasadena, California where the winner will ride on the Donate Life float at the Tournament of Roses Parade®* on January 1, 2009. The winners and their guests will fly coach class to California, be transferred to and from the airport, stay at a Pasadena-area hotel for five nights (12/28/08 through 1/1/09), and have up to $180.00 per day (total for both the winner and guest) for meals.
- 2. No purchase necessary. Contest open only to liver, kidney or heart transplant recipients currently taking Prograf® (tacrolimus capsules), and who are legal U.S. residents and 13 years of age or older. Entrants must be physically able to stand for at least four hours (the time required to ride on the float).
- 3. Participants must submit an entry form and a brief essay in English of no greater than 500 words describing how their life has been transformed by transplantation. Entries can be submitted electronically at www.TransplantExperience.com, but must not be received later than 11:59 p.m. EDT on June 13, 2008. Entries can also be sent by mail, preferably typed, to Astellas Ride of A Lifetime, 875 N. Michigan Avenue, Suite 3300, Chicago, IL 60611-1959, postmarked no later than June 13, 2008. Entries over 500 words will be disqualified. Additional entry forms may be obtained by visiting www.TransplantExperience.com. All contest winners will be notified via phone and mail; the winners will also be announced on www.TransplantExperience.com by July 31, 2008.
- 4. Essays will be judged by a four- (4) judge panel consisting of a transplant physician, transplant coordinator, transplant pharmacist, and an Astellas Pharma US, Inc. employee. Essays will be judged based on: (1) readability; (2) the compelling nature of the story; and (3) the degree of transformation as a result of transplantation. One winner per geographical region (Northeast, Southeast, Mid-America, Great Lakes and Western) will be selected. Winners will be selected by a unanimous vote.
- 5. Employees of Astellas Pharma US, Inc, located at 3 Parkway North, Deerfield, IL 60015 and their respective family members, agencies, affiliates, and subsidiaries are not eligible for participation.
- 6. Each prize is estimated at a value of $4,800.00.
- 7. Winners must sign and return a Release of Liability and Consent to the use of their names and/or likeness in advertising and participation in media interviews (where not prohibited by law) within 14 days of notification or prize will be forfeited and offered to an alternate winner. There will be no substitution or transfer of prizes by the winner, and prizes are not redeemable for cash. Guests must also sign a Release of Liability.
- 8. All submitted essays and photos become the property of Astellas Pharma US, Inc.
- 9. The winners agree that Astellas Pharma US, Inc. and its affiliates and their officers, directors, employees and agents, successors and assigns shall have no liability whatsoever for any injuries, losses, or damages of any kind resulting from acceptance, possession, misuse or use of the prizes.
