Astellas Ride of A Lifetime Contest Rules
- 1. Astellas Pharma US, Inc. is sponsoring a contest in which five (5) organ transplant recipients will win a trip for two (the winner and one guest) to Pasadena, California where the winner will ride on the Donate Life Rose Parade® float on January 1, 2011. The winners and their guests will fly coach class to California, be transferred to and from the airport, stay at a Pasadena-area hotel for five nights (12/28/10 through 1/1/11), and have up to $180.00 per day (total for both the winner and guest) for meals.
- 2. No purchase necessary. Contest open only to liver, kidney or heart transplant recipients currently taking Prograf® (tacrolimus capsules), and who are legal U.S. residents and 13 years of age or older. Entrants must be physically able to stand for at least four hours (the time required to ride on the float).
- 3. Participants must submit an entry form and a brief essay in English of no greater than 500 words describing how their life has changed as a result of transplantation. Entries must be accompanied by a photo of the entrant. Entries can be submitted electronically at www.TransplantExperience.com, but must not be received later than 11:59 p.m. EDT on June 25, 2010. Entries can also be sent by mail, preferably typed, to Astellas Ride of a Lifetime, 111 E. Wacker Drive, 11th Floor, Chicago, IL 60601, postmarked no later than June 25, 2010. Entries over 500 words will be disqualified. Additional entry forms may be obtained by visiting www.TransplantExperience.com. All contest winners will be notified via phone and mail; the winners will also be announced on www.TransplantExperience.com by September 15, 2010.
- 4. Each entry, whether mailed or submitted online, must be accompanied by a photo of the entrant. For entries submitted online, please include your photo within the text of your essay. For entries submitted via mail, please affix your photo to your essay. The photo may be used in publicity materials highlighting winners, including, but not limited to, media correspondence, Transplant Experience and Donate Life Float Web sites and Astellas internal and external communications. Submitted photos become the property of Astellas Pharma US, Inc. and will not be returned.
- 5. Essays will be judged by a panel based on: (1) readability; (2) the inspiring nature of the story; and (3) the life changes resulting from transplantation. One winner per geographical region (Northeast, Southeast, Mid-America, Central and Western) will be selected. The decision of the judges is final.
- 6. Employees of Astellas Pharma US, Inc., located at 3 Parkway North, Deerfield, IL 60015, its agencies, affiliates and subsidiaries, and their respective family and household members, are not eligible for participation.
- 7. Each prize is estimated at a value of $4,800.00.
- 8. Winners must sign and return a Release of Liability and Consent to the use of their names and/or likeness in advertising and participation in media interviews (where not prohibited by law) within 14 days of notification or prize will be forfeited and offered to an alternate winner. Guests must also sign an Affidavit and IRS Form W-9 so that the value of the prize will be reflected in their 2011 taxes. There will be no substitution or transfer of prizes by the winner, and prizes are not redeemable for cash.
- 9. All submitted essays and photos become the property of Astellas Pharma US, Inc.
- 10. The winners agree that Astellas Pharma US, Inc. and its affiliates and their officers, directors, employees and agents, successors and assigns shall have no liability whatsoever for any injuries, losses, or damages of any kind resulting from acceptance, possession, misuse or use of the prizes.
