Common mistakes
In most cases, a healthcare provider will submit claims on your behalf. If your claim is denied, speak with your insurance provider to find out why. Some common mistakes include:
- Omission of the healthcare provider's number
- Inaccuracies in or omission of diagnostic codes
- Transposition or omission of patient ID numbers
- Inclusion of an invalid patient ID number
- Inaccuracies in or omission of the patient's date of birth
- Inaccuracies in or omission of the patient's address (street address, city, state, and ZIP Code)
- Inaccuracies in or omission of procedure codes
- Claim not filed on time
- Date-of-service error
If you are submitting your own claims, be sure to check all documents for accuracy and completeness.
