Common mistakes

In most cases, a healthcare provider will submit claims on your behalf. If your claim is denied, speak with your insurance provider to find out why. Some common mistakes include:

  • Omission of the healthcare provider's number
  • Inaccuracies in or omission of diagnostic codes
  • Transposition or omission of patient ID numbers
  • Inclusion of an invalid patient ID number
  • Inaccuracies in or omission of the patient's date of birth
  • Inaccuracies in or omission of the patient's address (street address, city, state, and ZIP Code)
  • Inaccuracies in or omission of procedure codes
  • Claim not filed on time
  • Date-of-service error

If you are submitting your own claims, be sure to check all documents for accuracy and completeness.
